Welcome to the Community Assessment Survey
This survey allows Highland County to receive feedback about the health of Highland County from the individuals that live in our community. We will use this survey to help identify key issues in our community, and to develop programs and services to improve these issues. This survey is for Highland County residents that are 18 years of age or older. Responses to this survey are completely anonymous.
Once you have completed the survey, you will have the opportunity to complete a separate survey to enter into a prize drawing! That prize drawing survey will not be connected in anyway to health assessment survey. The prize winner is 100% randomly selected from eligible participants that take the prize survey.
