Monarch Meadows Nursing and Rehabilitation ranked as Adams County’s third largest nursing home in the first quarter of 2026, based on data from the Centers for Medicare and Medicaid Services (CMS).
The facility had 50 available beds, which is 20 fewer than the Adams County average in the first quarter of 2026. Monarch Meadows provided care for an average of 46.8 residents during this time frame.
The CMS gave Monarch Meadows Nursing and Rehabilitation an overall score of 3 out of 5, below the 3.2 average rating reported for nursing homes statewide in Ohio.
Eagle Creek Nursing Center held the top spot as the largest in Adams County that quarter, supplying 85 beds. Adams County Manor followed in second place, offering 74 beds.
Monarch Meadows Nursing and Rehabilitation reported no fines or penalties for the first quarter of 2026.

U.S. Census Bureau estimates show the older adult population in the nation is rising while numbers of younger residents decline. Between 2020 and 2024, Americans age 65 and older grew by 13%. “Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
